Requesting Rights Advice Visits

For use by physicians and other professionals
Most community hospitals in Ontario have designated the PPAO to provide rights advice in their scheduled psychiatric facilities. The Minister of Health and Long Term Care has also designated the PPAO to provide rights advice to all persons subject to community treatment orders who are not currently inpatients in psychiatric facilities.

Below are two forms that physicians and designated staff may use to request rights advice. Each form is fillable on your computer screen, but must be printed and then faxed to our office. Please provide as much detail as possible.

To request rights advice for persons who are inpatients:
